Where do I send my forms?
All forms listed on this page for Chicago River Schools Network and Adopt A River Schools, including annual repots, evaluations, and grant applications should be mailed, faxed, or emailed to Friends of the Chicago River to the attention of Mark Hauser, Education Coordinator.
River Monitoring Forms
Please fill out and turn in one of each of the following monitoring forms each time your school visits the Chicago River. The chemistry values should represent the average values found by all groups, and the macroinvertebrate survey should include all types macroinvertebrates found throughout the entire day.
Even when you do not do all three types of monitoring, please always make sure to always fill out the habitat survey. The habitat survey gives context to your chemistry or macroinvertebrate data.

Annual Reports
Every year each teacher must turn in a Planning Document at the beginning of the year (by Oct. 31) and a Service Learning Documentation Form at the end of the school year (by May 30). If you collaborate closely with another teacher or teachers at your school, you can turn in one form for all collaborating teachers. Just make sure to write all teachers’ names at the top of the page. Please use the following forms:

Annual Evaluations
Every year each teacher must turn in a Service Learning Evaluation Form and a Teacher Evaluation (by May 30). In addition, one classroom of students from each school should fill out the Student Evaluation.

Small Grants Program
Thanks to generous grants from the McDougal Foundation and Chicago Public Schools Service Learning and support of Friends of the Chicago River, each participating Adopt A River School can receive up to $300 for related expenses during the 2007-08 school year. Grants are awarded on a rolling basis.
